A: In Halseys journal it was revealed that Cortana was split, the incarnate travelling with the Chief and much of the intelligence was stationed under Sword Base analyzing the crystal found on Sigma Octanus.
When the Cortanas were once again merged after Keyes regained the other half of Cortana she directed Captain Keyes to escape Reach by jumping to the coordinates found in the crystal.
Makes perfect sense.
Q: Dr. Halsey didn't know about the Spartan III's WTFBBQ?
A: It was revealed in Halseys Journal that she spent much time wondering about that, whether it was a project piggybacking on the back of her sucess or another seperate project she did not know.
When she discovers the "SIII" line in Akersons files she was even more curious which only makes for a small retconn.
Q: The Pillar of Autumn was in docking in orbit around Reach, not in the shipyards?
A: This has been retconned, Marcus says it now docked on Reach not in orbit.
